The Tooth Taxi is a 39 foot state of the art mobile dental van that visits public schools throughout the State of Oregon treating children K-12. Our goal is to reach areas that have limited access to dental resources and students without insurance. During the summer months, and when these schools are closed, the Tooth Taxi serves primarily children at nonprofit organizations that meet our requirements, and as scheduling permits. A Tooth Taxi Site Partner Request Form must be submitted to be considered for a Tooth Taxi visit. Request forms must come from an authorized representative from the school district or school; such as the principal, teacher, or school nurse.

The Tooth Taxi brings dental care directly to patients by visiting schools and other community sites. Our professional dental staff assess each patient's oral health and provide comprehensive care. They also provide oral health education through classroom presentations; teaching youth how to prevent tooth decay and how a healthy mouth can affect overall health. The Tooth Taxi provides comprehensive dental care, free of charge.

Fill out the REQUEST FORM and submit it to the Tooth Taxi’s Project Manager.

2.

When filling out your request form, be sure to choose a primary contact person at your site to help organize the visit. This should be someone who is able to commit the time needed to prepare for the Tooth Taxi visit. If your site is selected, the Tooth Taxi Program Manager will work with you to coordinate a successful visit.

Identify the patients you want served. We rely heavily on you to tell us who needs help the most! We give priority to youth who have no dental insurance, have never been to a dentist, or have an urgent need.

Upon booking confirmation, you’ll be given consent forms to send home with prospective patients. Collect and prepare Tooth Taxi forms in advance of the Tooth Taxi visit, making them available to the Tooth Taxi Program Manager.

  • An up-to-date medical history signed by a parent or legal guardian and a signed consent from parent or legal guardian.

  • The Tooth Taxi visits schools that have a high percentage of students that qualify for the free and reduced lunch program. The Tooth Taxi gives treatment priority to children who have no dental insurance, children who have never been to the dentist, or children with an emergent need.

  • We will treat students in kindergarten through 12th grade who meet the other requirements, regardless of age.

  • The Tooth Taxi will provide basic dental care, including exams, cleanings, sealants, restorations, and extractions.

  • The Taxi is overseen by a dentist licensed in the State of Oregon.

  • The costs associated with the Tooth Taxi will be covered by the generous contributions from our donors.

  • Yes, we will offer to teachers a 20-minute oral hygiene presentation for their class. Students who receive appointments on the Tooth Taxi will receive one-on-one education.

  • Appointments generally last 30-60 minutes. Students will be escorted from the classroom to the Tooth Taxi and back to the classroom.
