The Tooth Taxi is a mobile dental van that visits public schools throughout the State of Oregon treating children K-12.  Our goal is to reach areas that have limited access to dental resources and students without insurance. During the summer months, and when these schools are closed, the Tooth Taxi serves primarily children at nonprofit organizations that meet our requirements, and as scheduling permits.  A Tooth Taxi Site Partner Request Form must be submitted to be considered for a Tooth Taxi visit.  Request forms must come from an authorized representative from the school district or school; such as the principal, teacher, or school nurse.

Requirements:

  1. Dedicated project coordinator to arrange and oversee the Tooth Taxi visit.  This should be someone familiar with the school system (staff personnel, retired personnel, parent volunteer, etc.) that has time to dedicate to Tooth Taxi visit preparations. The project coordinator needs to be assigned so the Tooth Taxi program manager and or OEA Choice Trust rep can schedule a pre-site visit meeting to review preparations for hosting the Tooth Taxi.
  2. High percentage of free & reduced lunch recipients
  3. School with adequate and safe parking for Tooth Taxi
  4. School agrees to arrange transportation for students coming from other school(s) in the district (Tooth Taxi parks at one site for one week).
  5. One volunteer per day to escort students to Tooth Taxi and back to class during Tooth Taxi site visit.
